Staff Reported Measure Now Open!
We encourage you to begin sharing the survey with members of inpatient staff who are likely to come into contact with dying patients and those important to them as soon as possible. Guidance and promotional materials can be found on our guidance page. The Staff Reported Measure element of the audit will close on Tuesday, 30th June 2026.
Unique links and QR codes have been generated for each submission registered for NACEL 2026. These can be found on your 2026 data collection pages within the Members' Area, below the Case Note Review. If your organisation has more than one submission, please ensure the correct link and QR code is used for each submission, as surveys should be collected for all submissions.
Please note that any submission with no Staff Reported Measure surveys received will be flagged as an outlier due to partial non-participation in line with the NACEL 2026 Management of Outliers Policy.
Please continue submitting data to all open data elements, including the Case Note Review and the Bereavement Survey. Ensure you are aware and meet all key dates.

Patient and Carer Tool information
Read more about the next phase of the Patient and Carer Tool, which will make hospital-level data publicly available
Background
The NACEL Patient and Carer Tool was published in 2025 to make the audit’s findings accessible to patients, carers and anyone interested in end-of-life care in hospitals at a regional or national level, this was Phase 1. This tool is now in development to allow the public to view data for individual hospitals, this is Phase 2. Data for England, Wales and Jersey is included within the Patient and Carer Tool, supporting transparency and quality improvement across nations. You can find out more about the Phase 2 development of the NACEL Patient and Carer Tool in the document linked below.
We recommend hospitals do the following:
Ensure your hospital board, communications team and patient/carer experience teams are aware of the date for release of your hospital level data.
Ensure you are familiar with your Organisational data or organisation's findings in the Data and Improvement Tool.
The January-December 2024 dashboard has a ‘Patient and Carer Tool’ tab which displays hospitals’ results for the 5 metrics at a regional level. The 2025 dashboard will be released by the end of this month.
Share your local NACEL data with governance structures / Board as soon as it is published.
Discuss the Patient and Carer Tool with the board and executives responsible for PEOLC in the hospital as well as at meetings you feel relevant e.g. ICB or Health Boards and Trusts (Wales) level meetings, End of Life Care groups.
Share your quality improvement plans relating to end of life care with your ICB/Health Board and Trusts as per NACEL 2024 National Recommendation 1.

Patient and Carer Data Tool Drop-In Session
In 2025, a newly developed Patient and Carer Tool was developed alongside a group of bereaved relatives, showcasing some of the NACEL data to the public, by region or country. This tool can be found here www.nacel.nhs.uk/patients-and-carers-data.
In 2026, the tool will be redeveloped and will allow the public to see NACEL data for their local hospital. This means that your patients and their loved ones, amongst others, will be able to view some of your data via the NACEL website, a publicly available platform. A lot of work has been going on with national, regional, and local stakeholders including members of the public to develop this tool. In the coming months, more communications will follow with specific recommendations for action at a hospital level.
Join us for a drop in session focusing on Phase 2 of the Public Facing Tool which will take place Tuesday, 12:30-1:30pm, 21st July 2026. Register below:
