Reminder to submit your NACEL 2025 data
Data collection is currently open for the Case Note Review and Bereavement Survey. Please continue to submit your data via the data collection pages.
The QR code and URL for the Bereavement Survey can also be found on the data collection page. If you have multiple submissions you will need to select the site from the drop-down menu. The QR code and URL are unique to each submission. To help disseminate the QR code and URL we have created a number of resources, which you can access here.
Data collection for the Hospital/Site Overview will commence on 1st July 2025.

Integrated Care Boards (ICBs) now have direct access to the NACEL Data and Improvement Tool.
This access should empower ICBs to take a more informed, system-wide approach to end of life care.
The purpose of providing ICBs access to the NACEL Data and Improvement Tool is to support targeted improvements in the quality of care provided to patients at the end of life. This access should empower ICBs to take a more informed, system-wide approach to end of life care. This change in approach has been agreed between NHSE and HQIP who are joint controllers for the collection and use of NACEL data.
